Banner Contact Form Ticket Template

You may use the below ticket template to request to add a contact form on your Website banners.

We are adding these not only on the homepage but also to all the Dynamic Pages, since these are the pages that are optimized to show up in the search engines.

Also, while we can use Cognito & Formsite on the Banners, it is advisable to just use a simple Duda Contact Form as we can't have a very big form or the banner becomes too vertically big, skewing the look of the banner.
